Override Rationale

Required before any escalated package can be approved. The rationale is a structured attestation — evidence reviewed, corrective action, and the approver's sign-off — and the approver must be different from the preparer (segregation of duties). In production, submitting writes this rationale into the binder's immutable, hash-chained evidence package.
